So, out of the ones I've seen, Raiders of the Lost Ark wins by a mile. This is mainly because of the gross-out scenes in The Temple of Doom. The feast in the palace was disgusting, and the room with all the bugs was just creepy. I can do snakes a lot better than bugs as far as yucky scenes go. (Except the snakes in Temple of Doom. That was just flat-out disturbing.)
Raiders of the Lost Ark was, however, unnecessarily graphic in my opinion. According to IMDb, the face-melting scene almost gave the movie an R-rating. Remember, back in those days there was no PG-13. But other than that, it was a great movie. The villains were good and met a fitting (although painful and terrifying) end. As a Christian, I do disagree with the portrayal of the Ark of the Covenant, but that doesn't really tarnish the movie. You have to admit, the desert chase was pretty cool.
Who do you think met a worse fate? Was it Belloq, Major Toht, Colonel Dietrich, or the German mechanic/boxer? My vote goes for Toht. His was the most graphic.

The Temple of Doom was one of the movies that encouraged them to make the PG-13 rating. Unnecessarily graphic?! It's not graphic AT ALL compared to current day action movies:S. BTW, it's PG, not R. Nowhere close to being R. Yes, it should have been PG-13, but there was no PG-13 at the time. Still, closer to PG than R for sure. You do realize R means NOBODY under 17 should be allowed to watch it, right? I saw Raiders when I was 10 and was fine:S. The face-melting was like the worst special effects ever. It was literally just stop motion. If you watch it again, you will see that the glasses randomly appear 5 inches lower on the face randomly in the middle of the quick scene:P. Overall, there have been 1,000 more graphic deaths in movie history than that one. I don't even think that was the most graphic in the movie, I think the German boxer's was MUCH worse. As for the last question, they all died so they all really have the same fate. As for the most painful, definitely the German boxer.

I also am a Raiders of the lost Ark favorite, I like crystal Skull but Indy was just to ol, i think they should have stayed with 3 but no one can seem to leave anything alone.

Yes, The Last Crusade is definitely a close second for those and many other reasons. The temple of the grail at the end is one of the best adventure scenes ever IMO!

I don't like Crystal Skull very much. Too much lens flare, too much Cate Blanchett, too much Shia Labeouf and too much CGI. Terrible CGI that is. In saying that, Shia isn't so bad. He's a Disney actor, and in a way, it's kinda what I expected from Indy's son. If you take out all the loudness, he would have been fine. Someone should have told him to play the role more like how River Phoenix played young Indy. Screaming isn't cool, it's not something a greaser or Indy would do... Unless, well you know snakes. It was an okay movie. I watch it about once a year, but that's probably because I watch the first three once a year.

Oh, I liked Cate Blanchett. Not that I liked her character, it was just interesting to see Galadriel so desperate for the Crystal Skull. And I didn't like Mutt very much. They were trying to pass the role down to Mutt, where he would start to become the main focus of the movies, but it didn't work. Everybody loves Harrison Ford as Indiana Jones, and that is irreplaceable. Indiana Jones is always everybody's favorite character.
