{JAMMERS! -Same!- ONLY!} Mmmmmmm, as far as I recall Ishamael, Aginor, Balthemal, Be'lal, Lanfear and Rahvin are dead Ishamael wasn't smashed in the first book, Rand just thought he was. He was however smashed in tGH at Falme. Aginor and Balthemal were smashed at the eye of the world.

{JAMMERS! back again} Actually, in Book 6 Rand said he smashed Ishamael at the Stone of Tear. So I think Ishamael survived the encounters in Books 1 and 2. Perhaps because of his tie to the Dark One.

Spoilers (again)} Okay, so you're probably tired of me tagging you for this by now, but there was a chapter with the Forsaken discussing this very thing, and we were right about Be'lal at the Stone. I guess Rand just assumed it was still Ishamael. So the full list of Forsaken smashed at this point is as follows:
Aginor, smashed by Rand, I think....? at the Eye of the World
Balthamel, smashed by the Green Man at the Eye of the World
Ishamael, smashed by Rand at Falme
Be'lal, smashed by Rand at the Stone of Tear
Lanfear, smashed by Moiraine at Cairhien
Rahvin, smashed by Rand at Caemlyn
Asmodean, smashed by a mysterious figure at Caemlyn
and also Moghedien, who's not actually smashed, but is being held captive at Salidar.
