Eh, that was Ishamael there, because he wasn't properly imprisoned/was imprisoned near the edge so he got free every 500 or so years. Those years coincided with the greatest events in the history of WoT, such as the Trolloc Wars, amongst other things. He is also free (Ishamael) at the start of the series, although he's a little more crazy by then (1000s of years imprisoned tend to do that )

Well, no, not really. The Breaking started right when the Hundred CONpanions led by Lews Therin Telamon sealed the Bore and the Dark One tainted Saidin. Those Hundred CONpanions all went mad instantaneously and thus began the Breaking. The Prologue takes place slightly after this, just after Lews Therin finishes smashing his family in his madness.

He truly thought he was the Dark One, or so it seems, and Rand seemed to think so too. I don't think Moriaine was sure he was the Dark One, she always seemed a bit mistrustful of Rand's belief

That is hard to say. I would say that it depends on how close the Dark One is to escaping his prison. If he were free, he would be much more dangerous. If he were completely sealed up, he would be much less dangerous. As he stands, sort of on the edge, it's hard to say.
